A Pre-Formatted Booklet

This a cerlox bound  booklet with a ½”  coil that is easily wound into place allowing for the owner to install/remove pages as they wish. There are colored see through dividers for each section. The pages are regular white 8.5” X 11” size with all printing in black.  The booklet/package contains enough pages to perform 15 new 4 day missions, with 15 HISTORY, 30 DIAGRAM, 45 SURVIELLANCE, 60 CONTINUATION, 15 DESTINATION, 15 PERSONS ID, 15 VEHICLE ID, 15 STATEMENT pages.

The pages can be personalized at owners request with the following:

  • Company name
  • Investigators name
  • Investigators license number
  • Modified abbreviations used in legend box

DESCRIPTION OF PAGES

SYNOPSIS AND HISTORY page

The top section contains boxes for starting and ending times including arrival,  travel times, mileage,  weather conditions including temperatures, wind, visibility, sky cover,  precipitation and accumulation at the start, during the day and for the evening. There is a legend of common abbreviations.  This sheet covers the initial or beginning of the file with lines/categories for all pertinent  info of the subject/s and for the history and details of the file including Incident, Injuries, Present condition, Possible vehicles to look for, legal representation,  treatment, and other. All lines are numbered for ease of reference. The column on the left side of page is for time of note or video taken.

SURVEILLANCE Page

Top section contains boxes for starting and ending times including arrival,  travel times, mileage,  weather conditions including temperatures, direction of wind and gusts, visibility, sky cover,  precipitation and accumulation at the start, during the day and for the evening. There is a legend of common abbreviations. The column on the left side of the page is for the time of the incident or video  taken.  The first line is for the mission of that day or any changes to be done.

 

CONTINUATION Page

Same as surveillance page without upper daily information boxes.  Used to continue the notes once the surveillance page becomes full.

 

 

DIAGRAM Page

This pages’ writing area is blank for diagrams, maps, etc.  It is intended mainly for a sketch of the immediate area of surveillance so that points can easily be referred to in notes.  IE. In notes after a description of the surveillance position or point of note one can mark a star and number it on the diagram and then use that point for further reference.  IE.  “1.dia.1, 2.dia.1 etc. or a house number and so on.

 

DESTINATIONS OF SUBJECT Page

The 2 left columns are for date and time with the remainder of the line for the destination the subject traveled to.

 

 

PERSONS I’D page

All lines are numbered with left columns for person identified IE.   Subject, male, female, dog etc. Which can be given a code IE.  Sj, M1, F1, D1 etc. The next 2 columns are for the date and time viewed. The remainder of the line is used to describe the individual.  As the file goes on these people now only have to be referred to by their ID code. The top of the page consists of boxes for the subject name, start and end dates of the file, File number, page number.  At the bottom of the page the investigator fills inputs the line numbers of the info. IE.  1-33 so that other info can not be added without the investigator knowing. There is box for the investigators signature and license number that can be added or hardcopied  to the page.

VEHICLE ID page

Very similar to the Persons ID format, a vehicle is given a code, then using columns for the plate number, date and time with the remaining of the line for a description.

 

 

STATEMENT page

Used for statement taking.
